PROCEDURES FOR SUPPORT COORDINATORS

Please make sure an updated tuition policy is signed and dated. It will assist families in understanding the process of submitting

Since The Achievement Center runs on a college semester schedule – FALL/SPRING/SUMMER, timeliness of submission will be important for student admission. Students that do not have an approved SDR will be required to pay privately in order to start their semester.

FALL SEMESTER Start date is September 1, 2021 for The Achievement Center

Step 1: Approval of Service Detail Report

Below is the information that you will need to complete the SDR

Procedure Code: T1999HI22

Unit Rate: Unit cost is generally $1050 per class however; other service options carry different costs. Please inquire.

Step 2: Provide the Approved SDR to The Arc.
